My answer to those questions is inevitably "The 'city watch' as it appears in most fantasy media is an anachronism; this city does not have a city watch or a standing army, only thugs in the employ of the rich who are concerned with protecting their own interests and not opening themselves up to attack from their rivals."

I tend to respond to that with "The worst thing that you could expect to come out of the wilderness in the real dark ages was a large mercenary army with no respect for human life. The stuff out there in a D&D world is considerably more terrifying."
