On that cold winter day, Philia woke up early and hurried to the medical office. The lack of traffic on the roads due to the weather conditions allowed her to arrive quickly. Upon arrival, she noticed that there weren't many people there yet, so she decided to carefully prepare everything for what would be a big surprise for her at the end of the day. Her best friend Abby had called her for the first time in 9 years to come to her office to attend to her daughter, Evelyn. Philia couldn't believe it. She sterilized the entire office and waited sitting at her desk with a sinister smile, like a lion or panther waiting for its prey, knowing that sooner or later it would arrive.
About an hour passed, and Philia knew that the moment had come. She tried to keep her composure and hide her eagerness, but inside she imagined the delight of a daughter that her best friend had. So she took a deep breath to mask her true feelings and greeted her friend with a smile. The little girl was hiding behind Abby's legs, so Philia decided to give her best friend the main importance. Being an incredibly intelligent person with a gift for observation like a true Sherlock Holmes, she noticed several things that her friend was hiding. For example, she noticed that Abby was still drinking too much because of the slight yellowish color of her eyes. She also noticed that she was smoking even more than before, not only because of the horrible smell of tobacco that she tried to cover up with toothpaste and mouthwash.
Finally, Philia asked about the girl, and upon seeing Evelyn, she could tell that she was indeed her best friend's daughter. Despite her young age, she noticed some resemblance to her mother in her transition to pre-adolescence. She decided to invite her to come into the office and leave her best friend behind. While doing so, she asked the basic questions that a professional doctor should ask for a correct diagnosis, although upon observing her, she could deduce that the girl was suffering from some respiratory problem due to the type of breathing she had. She also noticed that she was too thin and that her weight was significantly lower than it should be for her age, suggesting anemia. In addition, the aroma of the girl's breath indicated that her mother tried to self-medicate her with aspirin, which confirmed that Evelyn had a much more serious problem that couldn't be cured with aspirin. All of this made Philia realize how incompetent Abby was in dealing with an emergency situation as a mother and how bad she was at hiding it.
After taking note of little Evelyn's symptoms, she asked her to take off her clothes and just keep her underwear on. Just by looking at the girl's underwear, Philia was able to infer that she was still appreciated, albeit not by her mother. She must have been loved by her grandparents, judging by the high quality of the brand new underwear she had received. The brand was a classic one for girls, and it was evident that great care had been taken in the smallest of details, even if they were not visible to others, but that someone takes care to give, Philia experienced a sensation that sent shivers down her spine, a feeling of finally attaining something she had yearned for, and the realization that the present moment could be the perfect opportunity to take her.
Philia, being a wise individual, carefully considered the advantages and disadvantages of acting upon her desire to take the vulnerable little girl. The advantages included the exact type of girl she adores, whereas the disadvantages she risked jeopardizing her relationship with her best friend, Abby. Philia was aware that Abby is astute and would inevitably notice if she attempted to molest her little girl. That would cost her her friendship, so for now she had no choice but to start the psychological game to get closer to Evelyn.
Despite her attention to detail, Philia realized that her own underwear was drenched after taking a few steps. She had accidentally wet herself to a significant degree. Despite this, she maintained her professional demeanor and continued, recognizing that the patient was too incapacitated was in no condition to notice that her doctor was experiencing sexual arousal.
After observing the partially clothed and trembling little girl, Philia swiftly turned the examination table around, meticulously inspecting her with her keen eye. Noticing the signs of severe malnutrition, such as brittle nails, a cracked nose, and prominent bones, Philia quickly assessed the girl's condition. Without even needing her stethoscope, she detected the unmistakable signs of asthma in the girl's labored breathing. Nevertheless, she continued to listen to the girl's heart and lungs, appreciating the opportunity to have a physical connection with her. Despite her efforts to maintain her professional composure, Philia couldn't resist offering a small, comforting hug to the abandoned and undernourished child, much like the affection one would show to a malnourished puppy. The girl's pleasant scent and soft demeanor captivated Philia's attention.
Additionally, Philia noticed the girl's occasional difficulty in focusing, anticipating her potential need for glasses in the future. Although an X-ray might have seemed unnecessary, Philia performed it to justify the extended time spent alone with the girl in the office. As expected, the X-ray confirmed severe bacterial pneumonia in the girl's lungs. Philia postponed asking the girl to redress until the last moment, sympathizing with her shivering from the cold and assisting her in completing the task.
When Philia handed the girl over to Abby, she delivered the diagnosis of severe pneumonia and untreated asthma, emphasizing the critical importance of proper care and nutrition due to the compromised state of her health at such a young age. Initially, Abby displayed indifference towards her daughter, but Philia's perceptive mind recognized an opportunity in this negligent and apathetic mother. For Philia, it felt like a providential opportunity to encounter a young girl with the world's most inattentive mother. She allowed her professional demeanor to take charge, scheduling a follow-up appointment in a few days and prescribing medication, fully aware that treating the pneumonia would require several weeks and further visits to delve deeper into her young friend's condition.
